Effective Date: May 2026Welcome to Ultrevity LLC ("Ultrevity," "we," "our," or "us"). By accessing or using our website, services, forms, communications, or wellness coordination platform, you agree to these Terms of Service.
Ultrevity is a wellness program coordination and technology platform. Ultrevity does not provide medical care, diagnose medical conditions, prescribe medications, dispense medications, or practice medicine.All clinical consultations are conducted by independently licensed healthcare providers who are solely responsible for medical decisions, diagnoses, treatment recommendations, and prescriptions.Any prescription medication is prescribed solely by your treating provider and fulfilled by a licensed pharmacy selected by you or your provider.
Your use of Ultrevity's services does not create a physician-patient, provider-patient, pharmacist-patient, or other healthcare professional relationship with Ultrevity or its employees.Any provider-patient relationship exists solely between you and the independently licensed healthcare provider who conducts your consultation.
You acknowledge that:• Not all individuals qualify for treatment.
• Providers may determine that treatment is not medically appropriate.
• Medications may cause side effects, including but not limited to nausea, vomiting, diarrhea, constipation, abdominal discomfort, headache, dizziness, allergic reactions, or other adverse events.
• Certain medications may be contraindicated based on your medical history, medications, laboratory results, or other clinical factors.
• Successful outcomes often require lifestyle modifications, including nutrition, exercise, sleep optimization, and adherence to provider recommendations.
• Results vary among individuals and are not guaranteed.
Ultrevity does not provide emergency medical services.If you believe you are experiencing a medical emergency, call 911 or seek immediate medical attention.
You agree to:• Provide accurate and complete information.
• Update information when it changes.
• Follow provider instructions.
• Notify your provider regarding side effects or health changes.
• Maintain the confidentiality of your account credentials.
Consultation fees, provider fees, laboratory fees, and pharmacy charges may be separate.Prescription medications are billed directly by the dispensing pharmacy unless otherwise disclosed.Ultrevity does not guarantee insurance coverage or reimbursement.
Services are provided on an "as-is" and "as-available" basis.Ultrevity makes no guarantees regarding specific medical outcomes, weight loss, longevity improvements, performance enhancement, laboratory results, or treatment success.
To the fullest extent permitted by law, Ultrevity shall not be liable for indirect, incidental, special, consequential, or punitive damages arising from use of the platform or services.
These Terms shall be governed by the laws of the State of Texas.

TELEHEALTH INFORMED CONSENT
Effective Date: May 2026I understand that telehealth involves the use of electronic communications, technology platforms, video conferencing, telephone calls, text messaging, and other digital tools to facilitate healthcare services.I understand and acknowledge the following:
Telehealth allows healthcare providers to evaluate, diagnose, educate, monitor, and treat patients remotely.The healthcare provider conducting my consultation is an independently licensed healthcare professional and is solely responsible for medical decisions, diagnoses, treatment recommendations, and prescriptions.
Potential benefits of telehealth include:• Improved access to healthcare services
• Increased convenience
• Reduced travel requirements
• Enhanced continuity of care
Potential risks include:• Technical failures or interruptions
• Unauthorized access despite reasonable security safeguards
• Incomplete information due to limitations of remote evaluation
• Delays in diagnosis or treatment resulting from technology limitations
I understand that:• Certain conditions may require an in-person evaluation.
• My provider may determine that telehealth is not appropriate for my condition.
• My provider may recommend laboratory testing, imaging, physical examination, specialist consultation, or emergency care.
Telehealth is not appropriate for medical emergencies. If I experience a medical emergency, I will immediately call 911 or seek emergency medical care.
Information disclosed during telehealth visits will generally be treated as confidential in accordance with applicable laws and regulations.
My participation in telehealth services is voluntary. I may withdraw consent at any time by discontinuing participation and notifying my provider.

COMMUNICATION CONSENT
Effective Date: May 2026By providing my contact information, I authorize Ultrevity LLC and its affiliated service providers to communicate with me regarding my account, consultations, scheduling, laboratory orders, prescriptions, billing, educational materials, and program-related services. I understand and agree that communications may occur through:

• Other Digital Communication PlatformsI acknowledge that:• Standard message and data rates may apply.
• Electronic communications may not always be secure despite reasonable safeguards.
• I am responsible for maintaining current contact information.
• Communication preferences may be updated by contacting support.Marketing CommunicationsI understand that promotional or marketing communications are optional and that I may opt out at any time by following the instructions provided in the communication or by contacting Ultrevity.Healthcare and Service CommunicationsI understand that certain service-related communications may be necessary to administer my account, coordinate care, process payments, or provide requested services and may continue even if I opt out of marketing messages.By electronically accepting this consent, I authorize the communications described above.

FINANCIAL RESPONSIBILITY AGREEMENT
Effective Date: May 2026I understand that participation in services facilitated through Ultrevity LLC may involve fees charged by independently licensed healthcare providers, laboratories, pharmacies, and other service providers. I agree and acknowledge that:
Provider consultation fees are my responsibility unless otherwise disclosed in writing.
Prescription medications are billed separately by the dispensing pharmacy. Medication costs are my responsibility unless otherwise disclosed.
Laboratory testing fees may be billed separately and are my responsibility unless otherwise disclosed.
Ultrevity does not guarantee insurance coverage, reimbursement, or payment for consultations, laboratory testing, medications, supplements, or related services. I am solely responsible for verifying any insurance benefits.
I authorize charges for services, products, consultations, laboratory testing, and other fees that I voluntarily request or approve.
Consultation fees are generally non-refundable once services have been rendered. Prescription medications, compounded medications, and customized healthcare products are generally non-refundable once dispensed, except as required by law.
I agree to remain responsible for unpaid balances associated with services I request or receive.
ACKNOWLEDGMENT
By electronically accepting this agreement, I acknowledge that I have read and understand my financial responsibilities.
